Coded-card key controlled valve interlock systems for the prevention of catastrophic accidents

It would be difficult to name a particular industry that would not benefit from an SFC coded-card key interlock system.

As a general principle, it may be said that operations which are safe when performed correctly can have catastrophic consequences when performed incorrectly. The oil, gas and chemical processing industries generally have a disciplined approach to design and operating practice — usually governed by well-recognised international standards and enforced by regulatory authorities and certification bodies. While good practice begins with good design, both are ultimately hostage to the "human factor".

Modern process plants are highly automated and regulated by distributed software management systems which are simply monitored by ‘Production’ personnel – often remote from the physical location of the plant itself. Indeed, some operations such as pig launching or receiving procedures can
be effected in semi-automatic mode using push button controls (again often from a remote station).

Maintenance procedures however invariably involve human intervention and interrupt automated processes creating ‘abnormal’ conditions for the duration of the work. Loading or unloading of pig traps, changeover of pressure relief valves, turbine servicing (requiring suspension of CO2 Fire Deluge), coupling or uncoupling of hoses for loading or discharge of tanker cargoes all involve human intervention and are hostage to the possibility of operator error. Distributed control systems (DCS) cannot effectively regulate such (maintenance) procedures — the SFC Coded Card Key Interlock System can.

The obligation to adopt best practice is a fundamental of safety management – mechanical key safety interlocking is a technology that has evolved to offer sophisticated technical solutions to complex and hazardous process applications and which increasingly is being adopted by major OpCo’s worldwide for protection of their people, their assets and their surrounding environment’ SFC ‘Visual Alert’ Key Cabinets provide an effective and infallible management control system against unauthorised or inadvertent operation of interlocked
valves or associated process equipment by keeping the coded keys which initiate the operation of critical valves under secure supervisory control.

Controlling the sequence of events in which process activities are conducted has been achieved historically using a Permit to Work (PtW) accompanied by documented instructions.

However, this system is hostage to ‘human factors’ Distractions, failure to interpret correctly, ignorance of the system – all can lead the operator to make errors which can manifest themselves in industrial accidents of varying magnitudes.

Trapped Key Interlocks are simple mechanical devices which can be customised to implement a safe sequence of operation in any process activity.

In the following pages we show how our mechanical key interlock system ensures that pig trap valves are disabled and proven shut and the vessel is isolated, drained and vented before opening of the closure for loading or unloading of pigs, or, how the spare pressure safety relief valve is on stream before an in service relief valve can be isolated and removed, or, how we prevent a tanker from departing a loading/discharge station before the cargo hoses are disconnected.

These and any other work tasks executed by human intervention can be completely regulated by SFC’s coded-card key interlocks – simple and total mechanical reliability to prevent operator error or violation to protect your plant, the surrounding community and the environment. In addition to our range of high integrity coded-card key interlock safety products, SFC also offer a comprehensive range of valve and process equipment security products for high and low-criticality applications.

Seamless hot rolled steel tubes for all applications

V & M Tubes is committed to providing its customers with the best products and services available on the market, through an unique global approach supported by local capabilities. This commitment is backed by an annual production of up to 3 million tonnes of steel pipe and tube manufactured in 10 hot-rolling mills in Europe, Brazil and the USA. The company has 12 strategically positioned production facilities world wide for OCTG, a network of around 100 VAM licensee workshops in over 40 countries devoted to local support for the VAM Premium Connections, and the skilled professionals are working around the clock to provide first-class customer service.

Oil and natural gas are the main energy sources in our modern world. Their reservoirs can be found all over the world, and more often than not these resources are located deep in the earth or beneath the seabed. For the exploration and production of oil and gas from onshore or offshore fields, high-performance steel tubes are required to penetrate depths of several thousands of metres. The tubes have to withstand high mechanical stresses, high pressures, high temperatures and often aggressive media, too.

These seamless tubes for oil and gas fields (OCTG), known as drill pipe, casing, tubing and riser pipe, are supplied by the OCTG Division of V & M Tubes. Under the company name Vallourec Mannesmann Oil & Gas, we offer a comprehensive range of products, including API thread connections and VAM Premium Connections for higher requirements.

Seamless hot-rolled line pipe is used for the transportation of oil and gas in offshore and onshore fields. These pipes, produced by the Hot-rolled Tubes Division of V & M Tubes, have to withstand similar stresses as the OCTG products and are generally supplied to customers’ specifications.

Product range

Casing — in dimensions from 4 1/2" through 26", with API thread connections or with VAM Premium Connections

Tubing — in dimensions from 2 3/8" through 4 1/2", with API thread connections or with VAM Premium Connections

Drill pipe — in dimensions from 2 3/8" through 6 5/8", with friction-welded tool joints, according to API or in VAM design (e.g. VAM EIS and VAM TAURUS)

Riser — production, work over and drilling riser, matched to customers’ requirements, in dimensions up to 26"

Remotely monitor pipe thickness in critical infrastructure

UltraCorr® utilizes high sensitivity ultrasonic technology to provide nonintrusive monitoring of corrosion and erosion.

The instrument can be used to log multiple transducers from a common connection point, to provide periodic time and date stamped measurements of wall thickness, or for more critical applications. It can automatically collect and store continuous measurements from a single transducer.

UltraCorr® is ideally suited to form the backbone of an ICDA asset integrity system for pipeline operators. Sensors are permanently attached to the exposed surface in areas such as low lying spots, drips on other locations where corrosion should be monitored. After backfilling, the sensors are read from the surface by way of attached cables. The high resolution of UltraCorr® enables pipeline operators to detect changes in corrosion rates and readjust accordingly scheduled reassessment intervals to suit. Repeat excavations are unnecessary, and the guesswork about reinspection intervals is removed.

Typical measurement time per transducer is a few seconds. The user can program the UltraCorr® instrument to collect data from as many as 50 transducers, or alternatively, it can be programmed to collect up to 1,024 data points from one transducer at programmed intervals. Operation is made very simple by a series of user prompts shown on the LCD display. A rugged package along with extended battery life provides maximum versatility for the corrosion technician.

The software included with the instrument facilitates the transfer of data to a PC for analysis and permanent storage. UltraCorr® is also compatible with the CorrData® Plus (for Windows®) software used with the industry favorite CorrData® Remote Data Collection System.

Specialist trainers for the design, implementation and management of RBIM programs

Tischuk are specialist trainers for the design, implementation and management of RBIM programs.

RBIM Program Implementation, Facilitation & Audit

Courses are run periodically in Aberdeen and as required at client premises. Various options are available. The most popular approach is an initial week of RBI Practitioner training, equipping delegates to implement programs with the help of an experienced RBI Facilitator.

Once practitioners have gained experience, the Level 2 Facilitator course offers the next step, building on their knowledge and experience and adding the necessary tools for RBIM program management.

The final stage is the Tischuk Level 3 course, teaching the skills necessary for RBI program audit. All courses have certification options.

Our aim for the courses is to ensure delegates achieve a complete understanding of the concepts of risk and the practicalities of designing, implementing and managing effective risk-based programs, to achieve the benefits of optimised inspection & maintenance.

The courses cover all aspects including classroom learning, workshops and practical exercises using RBI software and real plant data, the calculation of risk, inspection/activity planning and work pack generation. Delegates see risk-based software in action and gain in depth understanding of how RBI can work best for them.

Delegates attending the practitioner course will undergo detailed training in the use of the latest version of the Tischuk 32-bit T-OCA software and have the opportunity to be assessed for certification in accordance with the requirements of API.

The audits provide a valuable check on the effectiveness of existing systems and programmes, allowing customers to benchmark against some of the most effective programmes in the world. The audit can also be used as the basis of a 3-stage programme of improvement and change implementation.

The auditors supplied are all highly experienced, practical engineers, assigned on the basis of their knowledge of customer’s plant type.

Auditors bring experience of integrity management from plants all around the world and are a vital component in helping customers leverage maximum benefit from their asset integrity management (AIMS) and RBI/RBM programmes.

Customers may also use auditors’ presence on site to train any of their newly-appointed integrity team members, or to provide refresher training to existing team members. If customers are also using the Tischuk risk-based technology, this may have been upgraded and the auditor can also be used to train customer staff in the use of these upgrades.

Another component of the service is linked to Tischuk’s RBI level 3 Auditor Training.

While Tischuk is a strong supporter of external auditing, they also believe internal auditing is essential to the health of any system or program. They therefore provide classroom-based auditor training. This can be supplemented by support of our auditors on site, working through an audit program with client internal auditors.
